2009-2010 Cosmetology Memo of Understanding

COSMETOLOGY PROGRAM

STUDENT MEMORANDUM OF UNDERSTANDING 

           

            The Grundy Area Vocational Center contracts with the Morris Academy of Hair Design to provide training leading to a state examination required for certification as a cosmetologist.  The training is 1500 hours in length and is held at the Morris Academy of Hair Design located at 100 W. Commercial Drive, Suites 6 & 7, Morris, Illinois.

 

  I.        Students who wish to participate must meet the following criteria:

            A.        Be a Junior or Senior in a school which is a member of the Grundy Area Vocational Center.

 

            B.         Be enrolled through the Grundy Area Vocational Center.

 

            C.         Be at least 16 years of age by December 1st of the year of enrollment.

 

            D.        Complete an application with the Morris Academy of Hair Design and sign a contract with that institution.

 

 II.        Students will be responsible for the following items of expense:

            Payments to the Academy:

            A.        First Year in Program  (750 hours)

                        1.         $  100.00  One time enrollment fee (non-refundable)

2.         $  800.00  For beautician kit and books which become the property of the student.

3.         $  400.00  For Lab Fees & Supplies

                                                $1,300.00  (Total amount is paid to Morris Academy of Hair Design)

                                                                        (Due by June 30, 2009)

NO PERSONAL CHECKS

                        4.         Supplies and materials as necessary (you will need to purchase throughout the year)

                        5.         Reliable transportation to and from Academy.

                        6.         Purchase of uniform and shoes, if needed.

 

            B.         Second Year of Program  (750 hours)

                                    1.         Supplies and material as necessary.

                        2.         Reliable transportation to and from the Academy.

                        3.         Purchase of uniform and shoes, if needed.

            A junior or senior who completes the first 750 hours of the Cosmetology Program and decides not to complete the second 750 hours is still liable for paying the Morris Academy of Hair Design the amount of money designated in the contract he/she has with the Morris Academy of Hair Design for the second 750 hours.

            ****Students who enter the program as seniors will be responsible for paying for all hours in excess of 750 hours.

            First year students who do not have 750 hours of instruction by May 31, 2010, will have until June 15, 2010, to obtain the deficient hours without additional cost.  After June 15, 2010, any additional hours of instruction must be paid to the Academy at the rate of $3.50 per hour.  Students must have 750 hours of instruction completed prior to July 31, 2010.  Students dismissed from the program as a result of not meeting the 750 hour requirement will still be liable for paying the Morris Academy of Hair Design the amount of money designated in the contract the student has with the Morris Academy of Hair Design.

            After May 31, 2010, any additional hours of instruction must be paid to the Academy at the rate of $3.50 per hour, for second year students who have not completed 1500 hours.

 

III.       Other Student Responsibilities

 

            A.        To be at the Academy on time

 

            B.         Parent/Guardian is to notify both their student’s home school and the Academy at 942-5662 when their student will be absent.

 

            C.         To follow the rules of both the Academy and the GAVC

 

            D.        Students are to attend classes on Saturday at the Academy.  Failure to do so will result in disciplinary action taken by the Grundy Area Vocational Center.

 

IV.       Days, Times of Attendance, and Tardies

      

            Students will be scheduled either 7:30 - 11:30 or 12:30 - 4:30, Tuesday through Saturday.  The Academy is closed on Sunday and Monday.  A student will be counted tardy if they are not at the Academy at the designated starting times.  Since there are 180 days in the school year and only 4 hours per day are scheduled, it is necessary for students to attend extra hours on Saturday and/or during school vacations such as winter break and spring break in order to obtain the required 750 hours.  If a student fails to attend a class on a Saturday, without following procedure, they will not be allowed to attend the following Tuesday.  The Saturday attendance policy is detailed in the Rules and Regulations.

 

V.        Criteria for Success

 

            1.         Prepared and positive to provide customer service as assigned.

            2.         Students will come to class to participate and remain on task for the session.

            3.         Regular attendance.

            Subject to the approval of the Center, the Academy shall have  the  right to  expel a  student  who is not  meeting its academic standards, or who is not adhering to the rules and regulations of the  Academy,  the  Academy  shall  give to the Center, and the student expelled, written notice stating the reason for the students expulsion and the effective date of the expulsion not less than seven (7) days prior to the effective date.  In the event that the Academy, with the approval of the Center, expels any student, the Academy shall bill the Center the prorate tuition at the rate of $3.50 per unused but unpaid hour, for the remainder of the current school year.  We the undersigned understand the requirements for enrollment and participation in the Cosmetology Program of the Grundy Area Vocational Center.  We wish to participate in the program and agree to the terms as set forth in this memorandum.
